Identification Requirements

For office visits, you must bring specific forms of identification with proof of date of birth, social security number and residential address. Be ready, and bring the right stuff.

Important: You cannot become REAL ID'd online! You must come in to a service center with the appropriate documentation.

U.S. Citizens

Identification

Bring one (original or certified copy):

  • Certified U.S. birth certificate (not a hospital issued birth certificate)
  • U.S. passport in current name
  • Certificate of naturalization
  • Consular proof of birth abroad

Additional required documents if you’ve legally changed your name:

  • U.S. passport in current name OR
  • Original or certified copies of all marriage certificates or court orders that show your name change(s) history (uncertified photocopies are not valid)

 
Social Security Number

Bring one (original):

  • Social Security card
  • W-2 form
  • Paycheck stub showing full Social Security number
  • 1099 Form

 

Residential Address     

           (not post office box)

 

Bring two:
(Remember, both of these documents must be in your name.)
